Raven attended McNeese State University and Illinois State University. Raven joined Physician's Choice Physical Therapy in March, 2005. She currently serves as the Athletic Trainer in charge of Brusly High School Athletics. She is a native of Lafayette, Louisiana and resides in Baton Rouge. She received her Bachelor of Science in Health Promotion with a concentration in Athletic Training from McNeese State University in 2002. Raven was a student athletic trainer for the McNeese Cowboys for 2 1/2 years. She completed her Master's degree at Illinois State University in 2004. Raven was a teaching graduate assistant in the Department of Kinesiology and Recreation at ISU.Melody A. Hammons, MPT
Melody attended the University of Mississippi Medical Center and graduated in 2005. Melody joined the staff at Physician's Choice Physical Therapy as a physical therapist in July of 2005. She is a native of Vicksburg, Mississippi and now resides in Walker, Louisiana. Melody received her Bachelor of Science degree in Biology at Mississippi State University in 2003. She then went on to complete her Master's degree in physical therapy at the University of Mississippi Medical Center in May, 2005.Tejas Vora, DPT

J. Blaine Callender, LOTR, MOT
Blaine graduated from the LSU Health Sciences Center in Shreveport in 2005. He joined the staff at Physicians' Choice Physical Therapy in October of 2005 as an Occupational Therapist. Blaine is a native of Denham Springs where he also attended high school. He received his undergraduate degree in Kinesiology with a concentration in Human Movement Science from LSU in 2003.

Abby Tullos, M.S., CCC-SLP
Abby joined the staff at Physician's Choice Physical Therapy as a speech-language pathologist in August of 2006.
She is a native of Monroe, Louisiana and now resides in Baton Rouge, Louisiana. Abby received her Bachelor of Arts and Science degree in Communication Disorders at Louisiana State University in 2002. She then went on to complete her Master's degree in Communicative Disorders at the University of Louisiana at Monroe in December of 2005. Abby is also certified as a VitalStim® provider.
